CSV Format Guide
Your CSV must have these columns (names are flexible — exact matching not required):

In MT4: Account History tab → right-click → Save as Report → save as CSV or xlsx. In MT5: History tab → export to CSV or Excel. You can also drag in an Excel file directly without converting.
